Join to apply for the Children's Community Nurse role at Cambridgeshire Community Services NHS Trust.
We are looking for Children’s Community Nurses to join our dynamic team, based out of The Oak Tree Centre in Huntingdon and The Peacock Centre in Cambridge. The team covers the whole of Cambridgeshire and provides a seven day service, delivering high-quality care to children and young people requiring nursing care in their homes or other community settings. This role supports early discharge from hospital, prevents hospital admissions and ED attendance, and aims to make a difference in children’s lives.

Responsibilities
* Deliver a high standard of nursing care in the community, in clients’ homes or other community settings, with the ability to travel effectively and independently across Cambridgeshire.
* Facilitate early discharge from hospital and help prevent unnecessary hospital admissions and ED attendances.
* Work as part of a multidisciplinary team with education, health, social care and voluntary sector partners.
* Utilise your initiative while also collaborating effectively as a team member.
* Participate in induction and ongoing professional training to develop expertise within the team and as an individual.
Qualifications
* RSCN/RN Child qualifications (as applicable).
* Excellent communication skills and the ability to work autonomously and as part of a team.
* Experience in community nursing is preferred; experience working with other agencies including education, health, social care and the voluntary sector is advantageous.
* Must be able to travel daily across the geographical area; reasonable adjustments considered under the Disability Discrimination Act.
