The Rehabilitation And Frailty Team (RAFT) provides daily input for up to 21 days in the patient’s own home. The service hours are 8am – 8pm with a care element added 7am – 10pm 365 days a year. You will be working occasional weekends and bank holidays on a rota basis. Car driver essential.

Are you a band 6 or an experienced band 5 looking for an exciting new opportunity? We are seeking a skilled, motivated, and dynamic Nurse with a passion for high quality care and service development.
Responsibilities
* Manage your own clinical caseload of community patients.
* Support the HCA/rehabilitation assistants with training, supervision, and joint sessions.
* Work as part of the Rehabilitation And Frailty Team (RAFT) to deliver community care.
* Contribute to service delivery and development to avoid unnecessary hospital admissions where safe to do so, with onward referrals to other community services including health and social care.
* Be involved in a wide variety of patients (neurological, orthopaedics, falls, frail, end of life, admission avoidance, long-term conditions, musculoskeletal) to gain broad experience across specialties.
Probation and Induction
* This post is subject to a probationary period. Internal applicants are exempt from probation (unless they are currently part way through a probationary period or are a current bank member of staff).
* NELFT places importance on induction; new starters undergo a comprehensive induction of up to two weeks, including mandatory training and systems training.
Location, Hours and Benefits
* Hours: 8am – 8pm with some on-call elements; weekends and bank holidays on a rota basis.
* High Cost Area Supplement – 5% of basic salary, with a minimum payment of £1,303 and a maximum payment of £2,198.

* Car driver essential.
* Experience at band 5 or band 6 level in community nursing.
