Job Overview
An exciting opportunity has arisen for an assistant practitioner to join our Community Rehabilitation Teams in Kirkcaldy. The Community Rehabilitation Teams provide a service to the adult population within the various localities. This service is delivered to service users within their own homes or, when deemed essential, as an out‑patient.
Responsibilities
The primary function of this role is to work with the Occupational Therapy and Physiotherapy staff as part of the Community Rehabilitation Team to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital. The post holder will also be expected to hold their own caseload, assess, progress and discharge autonomously within parameters.
Work may involve visiting other community hospital sites. Public holiday working is in operation. The role requires the ability to travel throughout the area.
Qualifications and Experience
* HNC/HND in Health and Social Care or equivalent or SCQF level₈ (e.g. Postural Stability Instructor) or equivalent;
* Experience working as a therapy support worker;
* Experience working with older people, as they are the main group of service users referred.
Skills and Attributes
Working as part of a supportive multidisciplinary team you will be expected to have excellent organisational skills, excellent communication skills and be able to work independently as well as within the team.
Working Conditions
Travel throughout Fife is essential. Some working may be in other community hospital sites. Bases are to be determined; it would be useful to note areas of work preference on the application (e.g. Dunfermline locality, Glenrothes locality).
The service offers regular supervision, PDP and supports training opportunities.
