KCHFT are looking to recruit a Pharmacist who is seeking a development opportunity. This role supports a competent 8a pharmacist to gain leadership and strategic skills to progress, within a maximum of 18 months, into an 8b pharmacist who can demonstrate leadership competencies against the NHS Leadership model. A personalised development programme will be agreed with the post holder to meet a competency assessment within 18 months for the 8b role. During development you will be mentored by the Deputy Chief Pharmacist and supported to lead the pharmacy frailty enablement teams within the community hospitals and domiciliary settings (Integrated Pharmacy Team – IPT). These are two teams of pharmacists, technicians and pharmacy support workers, one in each setting.
You will develop and implement a highly specialised integrated clinical pharmacy frailty service within these settings, ensuring the team works to the same aims with a patient- and MDT-centred focus. This role requires collaboration with external partners such as Healthcare Partnerships (HCPs), Integrated neighbourhood teams (INTs) and PCN MDT members, and a flexible approach to evolving NHS landscape.
Main duties
* Lead the pharmacy frailty enablement teams within the community hospitals and domiciliary settings (two teams).
* Develop and implement an integrated clinical pharmacy frailty service with a consistent patient and MDT focus.
* Develop collaborative relationships with internal KCHFT clinicians and frailty service leads to identify where the team can add most value.

Person Specification
Qualifications (Essential): MPharm / Reg GPHC / Postgraduate pharmacy diploma
* Experience of leading a team
* Working in an MDT to manage frailty within the last 2 years
