We are looking for an enthusiastic,committed and motivated Pharmacist to join the team and are seeking someonewho ideally has previous experienceof working in general practice, or alternatively experience of working inhospital, primary care, community or mental health pharmacy settings for atleast 2 years post-qualification, to work with our existing pharmacistto develop and lead medicines optimisation services within our practice.
The successful candidate will have apatient facing role in managing long-term conditions; working directly withpatients to assess and treat conditions, as well as promote self-care. Youshould have excellent written and verbal communication skills, experience ofinfluencing others and conducting clinical medication reviews.
You will work as part of amultidisciplinary team with clinical support to perform face to face andtelephone reviews to optimise medicines for patients with polypharmacy. Youwill manage the practice repeat prescription process, provide medicines informationadvice, perform medicines reconciliation upon transfer of care between settingsand promote systems for safer prescribing. The successful candidate will alsocontribute to quality improvement and clinical audit.
The applicant will be registeredwith the GPhC, ideally have a post-graduate clinical diploma and be anindependent prescriber.
Candidates must have strong written and verbalcommunication skills and an ability to work effectively under pressure.

3. Use appropriate provided clinicaltemplates within the EMIS Web Clinical System
4. Medicines reconciliation ontransfer between care providers

Lead on prescription management(systems and processes, ensuring monitoring is being done, supporting clericalstaff)

Person Specification
Experience
* Minimum of two years experience as a pharmacist, demonstrated within a practice portfolio
* Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice
* Demonstrates ability to integrate general practice with community and hospital pharmacy teams community groups
* Experience of working in GP practice.
Qualifications
* Mandatory registration with the General Pharmaceutical Council
* Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
* Membership Primary Care Pharmacy Association (PCPA)
* Membership of the Royal Pharmaceutical Society
* Minimum of two years experience as a pharmacist, demonstrated within a practice portfolio
