Hospital at Home (formerly Frailty Virtual Ward)
These posts will cover core working hours will essentially be daytime working. There will however be some requirement to work some weekend mornings and Bank Holidays. We are looking for individuals with a keen interest in developing a career in Elderly Medicine, General Practice or Palliative Care. All posts offered will be 12 months in length.
This role offers the opportunity to help shape the future development of the new and emerging Hospital at Home service within York and Hambleton, providing acute care to patients within their own home environment, supporting early hospital discharge and admission avoidance. There is expected to be further expansion into North Yorkshire.
Within your role, you will provide comprehensive geriatric assessment, diagnostics, medical intervention and referral to specialty colleagues as appropriate. Hospital at Home is a Geriatrician led, multidisciplinary team, with integrated pathways and collaborative working across acute and community services. Patients receive hospital-level care in their usual place of residence, including care homes.
The post holder will be based in York and will require the ability to travel by car across the community as necessary.
Selby Urgent Community Response (UCR)
This is an exciting opportunity for any resident doctor with an interest in community working. UCR is a service that provides a home visit response within 2 hours to frail older people in crisis. Crises can be medical or non-medical. The post holder would be required to provide the medical UCR response on a 9-5 Monday-Friday basis, based with the Selby Community Response Team in Selby Hospital. Patients will be assessed in their usual place of residence and conduct point of care diagnostics as needed. Patients are either then referred back to their own GP for onward care, or escalated as appropriate. Clinical supervision is provided by the Hospital at Home Consultant. Again, the post holder needs to be able to drive and travel to visits by car across Selby and surrounding areas.
** Successful Candidates are expected to hold a UK Driving Licence and access to suitable vehicle with business insurance.
Main duties of the job
The post holder will be expected to assess patients in their own home environment while also working collaboratively within the MDT to devise holistic management plans.
Day to day duties include triaging referrals, carrying out comprehensive geriatric assessments, performing diagnostics, creating treatment plans and referring to specialty colleagues as appropriate.
We are looking for a friendly, enthusiastic, and innovative individual who will work well with the team to meet service demands. We are seeking individuals with a keen interest in frailty and wanting to develop their skills and expertise with a view to formal training in geriatrics or as an extended role in general practice.
As this is a new and expanding service, there will be numerous opportunities for the post holder to get involved in quality improvement projects to further develop with Hospital at Home and Urgent Care Response services.

Qualifications and experience

26. Full registration with the GMC at time of starting the post
