An exciting opportunity has arisen to join the Home First Therapy team in Buckinghamshire.
We are looking for motivated and skilled individuals to join the team, which supports patients that have recently been discharged from hospital, to regain their independence and remain in their own homes.
The ideal candidate will be able to take a holistic and person centred approach to supporting patients to recover from their recent hospital admission, and to achieve their short term goals, maximising independence.
The role will include visiting patients in their home, providing therapeutic treatment of patients, under the guidance and supervision of qualified therapists within the team.
Main duties of the job
To be an integral member of the Home First Therapy team, providing prompt assessment and delivery of therapeutic treatments to patients in liaison with senior Therapy staff.
To take responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients own home.
To act as a technical resource to the Home First Service, in relation to the provision of mobility equipment.
To document, evaluate and liaise with qualified Physiotherapists and Occupational Therapists following interventions.
To be responsible for the completion and potential delegation of non-clinical tasks within Home First therapy team.
To work as part of the weekend Home First Service

EDUCATION, QUALIFICATIONS & TRAINING.
Essential criteria
1. NVQ Level 2 or 3 in Health and Social Care
2. Commit to undertaking Foundation Degree and any associated training once available.
Desirable criteria
3. Moving and Handling/First Aid Qualification
EXPERIENCE
Essential criteria
4. Significant experience working within a patient facing environment with a reablement/ rehabilitation focus
5. Previous experience working with patients with physical, cognitive or behavioural impairment
SKILLS, ABILITIES & KNOWLEGDGE

6. Good knowledge of the audit process
7. Collaborate with multi professional service users, internal and external at all levels.
8. Able to motivate self and be able to work with all levels of staff

12. Clear, concise verbal and written communication to ensure liaison with multi disciplinary team, patients and carers to ensure records and reports are accurate, legal and in legible manner (in English).

15. Ability to learn/ability to self initiate learning (evidence within the last 2 years)

SPECIAL CIRCUMSTANCES
Essential criteria
26. Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire.
27. Ability to work weekends/Bank Holiday
