We are seeking an experienced Practice Nurse to join our friendly and wellestablished clinical team. You will play a key role in delivering highquality, patientcentred nursing care and supporting the management of longterm conditions and health promotion across the practice population.
Responsibilities
* Deliver evidencebased nursing care in line with NICE guidelines
* Manage chronic disease clinics, including respiratory conditions (asthma/COPD)
* Provide wound care, including compression therapy and Doppler assessments
* Undertake cervical cytology, immunisations, and health checks
* Diagnose and manage acute and chronic conditions within scope of practice
* Prescribe under PGDs and review patient medications
* Support Infection Prevention & Control processes, audits, and compliance
* Maintain accurate clinical records and ensure effective clinical coding
* Support safeguarding of adults and children
* Assist with minor surgery when required
* Liaise with external services to support vulnerable and housebound patients
Qualifications
* Registered Nurse with current NMC registration
* Previous experience working as a Practice Nurse or in Primary Care
* Competence in immunisations, cervical screening and chronic disease management
* Strong communication and organisational skills
* Ability to work independently and as part of a multidisciplinary team
* Confident IT skills and experience using clinical systems
* Understanding of confidentiality, safeguarding and professional governance
* Qualification or experience in asthma and-or COPD management
* Wound care and compression therapy experience
* Nonmedical prescribing qualification (or working towards)
* Experience supporting Infection Prevention & Control initiatives
