Community Support Worker – Band 3
Grade: Band 3 – Contract: Permanent (Full time or Part time)
Hours: 36 hours per week (Full time) or 24‑36 hours per week (Part time)
Shift patterns: 5pm–11pm and 7pm–8am, requiring a combination of both shift types.
Location: Nottinghamshire Healthcare NHS Foundation Trust – Park House Health & Social Care Centre, Town Carlton
Salary: £25,760 – £27,476 per annum (pro rata for part time)

We are seeking an experienced and dynamic Healthcare Support Worker to join the NNE Heart Failure team in delivering high‑quality, patient‑centred care.
Main duties of the job
Provide support to patients in the community, working independently to manage your own working day and make appropriate clinical decisions. Collaborate with multidisciplinary teams across Nottinghamshire and partner services.

Qualifications
* NVQ III in Health and Social Care, Care Certificate or equivalent experience
Skills
* Ability to work as part of a team
* Good written and oral communication skills
* Diplomatic, caring and sympathetic approach
* Flexibility and adaptability
* Developed observation skills with the ability to report and record physical/mental health observations of incidents or events and behaviours accurately and in a timely manner
* Venepuncture experience
Experience
* Experience of working in a healthcare setting
* Experience of working in the community
Knowledge
* Basic understanding of different types of physical illness and a caring approach to patients
Training
* Willingness to undertake training that will enhance service delivery
