Overview
Band 6 Cardiology Specialist Nurse 45hrs a week available - Permanent. The Cardiology Nurse Specialist will form part of a cross-site team created to enhance the care of patients with newly diagnosed or chronic heart failure and patients with cardiovascular conditions. Also provide expert nursing care for patients with ischaemic, congenital and valvular heart disease undergoing potentially high risk invasive and non-invasive procedures. The successful candidates will be part of a wider team of Cardiac Specialist Nurses increasingly working collaboratively across primary and secondary care.

Working across two hospital sites in Carlisle and Whitehaven, the post holders will manage an individual patient caseload and advocate seamless care for cardiac patients, contributing to timely and appropriate assessment and management of newly diagnosed and chronic cardiac patients.

Responsibilities
* Provide expert nursing care for patients with acute and chronic heart disease undergoing high risk invasive/non-invasive investigations and procedures.
* Contribute to the management and development of inpatient and outpatient heart failure/cardiovascular nursing services across north Cumbria.
* Reduce unnecessary hospital admissions and readmissions through timely assessment and appropriate management of heart failure patients and those with cardiovascular conditions.
* Manage an individual patient caseload and advocate seamless care across hospital and community settings.
* Deliver specialty-specific education and training for patients, carers, healthcare trainees, and all MDT members across primary and secondary care.
* Participate in data collection for the national heart failure audit and other relevant audits/registries.
* Support delivery of NICE-aligned guidelines for Heart Failure with in-reach work to ward areas and outpatient clinics.
* Collaborate with a wider Cardiology Team across primary and secondary care to implement national best practice.
Requirements / Qualifications
* Registered nurse with relevant cardiology/heart failure experience or specialist training (Band 6 level).
* Ability to manage an individual patient load and work across hospital and community settings.
* Willingness to participate in audits, education, and multidisciplinary team activities.

Two hospital sites: Carlisle and Whitehaven. 45 hours per week. Permanent contract.
