Overview
An experienced Health Care Support Worker (HCSW) will join the Community Rehabilitation Team in the St Andrews locality. The Rehabilitation Service delivers care to adults as in‑patients, out‑patients, or in patients’ homes. This role works closely with the Occupational Therapy Service to deliver therapeutic treatment programmes in ward settings or at patients’ homes, supporting early discharge and reducing hospital admission.
Responsibilities
* Work with the Occupational Therapy Service to deliver therapeutic treatment programmes to service users in ward settings or at their homes.
* Carry out ongoing treatment programmes for a mixed caseload, mainly older people.
* Support early discharge and prevent hospital admission.
* Collaborate within a multidisciplinary team and provide treatment in other community hospital sites as required.
* Maintain accurate documentation and participate in regular supervision.
Qualifications & Requirements
* SVQ Level III in Health and Social Care or equivalent.
* Experience as a Physiotherapy and/or Occupational Therapy support worker (desirable).
* Experience working with older people (desirable).
* Good organisational and communication skills, able to work independently and within a team.
* Willingness to travel throughout the area and work on public holidays.
