We are recruiting for a Dual Diagnosis Practitioner at HMP Brixton.
Here at Forward Trust, we deliver a complex range of drug and alcohol services in the unique prison environment. Our support includes providing advice, health and wellbeing, motivational work, clinical services, and a wider range of group work and treatment programmes.
The Forward Trust services delivered within prison settings are commissioned by NHS England and are provided in partnership with primary healthcare providers and HMPPS. Integration and partnership are core to our work.
We believe that everyone can live a fulfilling life, regardless of their past. Our prison work aims to support those affected by drug or alcohol issues to create lasting change and reduce dependency, homelessness, unemployment, and reoffending.
Role Responsibilities
* Manage a caseload of service users, ensuring effective coordination of care between substance misuse and mental health services.
* Monitor engagement and facilitate effective sharing of information between services.
* Develop robust referral pathways and eligibility criteria for the Dual Diagnosis pathway.
* Co-design and co-deliver group work and interventions with relevant agencies and partners.
* Attend multidisciplinary meetings (MDT) or other relevant meetings for pathway development.
* Deliver training to the team and wider stakeholder groups. Possible line management and supervisory duties may be involved.
The post holder will promote recovery from drugs and/or alcohol dependence, supporting client goals such as reducing reoffending, increasing reintegration into meaningful occupations, and improving social and family relationships.
This role involves working across healthcare and substance misuse services to ensure the service meets contractual obligations, including timely data submission and performance management.
Some flexibility may be required for evening or weekend work on a rota basis.
The post holder will be directly line-managed by the service manager, with clinical supervision provided by service clinical leads tailored to individual needs.
All prison-based roles require an enhanced DBS check and HMPPS security vetting, which can take 3-6 weeks. All offers are contingent upon receiving these clearances.
