Key Responsibilities
* Perform clinical screening for inpatient and outpatient Systemic Anti‑Cancer Therapies (SACT)
* Provide expert clinical pharmacy support to haematology wards
* Work collaboratively with a multidisciplinary team to deliver safe and effective treatment plans
* Conduct final accuracy checks and release SACT doses produced within an aseptic unit
* Complete drug histories and medicines reconciliations for ward patients
* Support safe and well‑coordinated discharges through effective patient communication and counselling
Requirements
* GPhC registration as a Pharmacist
* Valid driving licence for travel between sites
