Job overview
Nursing associates are a new role within the nursing team in England, designed to bridge the gap between healthcare assistants and registered nurses. You will have established experience of providing hands-on, person-centered care in various healthcare settings, working with patients of all ages. You will be registered and regulated by the NMC and have your own standards of proficiency.
This position offers the opportunity to work flexibly and remotely in supporting Personalised Healthcare Commissioning services to deliver Continuing healthcare.
You will play a key role in supporting the Continuing Healthcare service across the region. Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and client-focused service.
You will assist in facilitating multi-disciplinary assessments and patient reviews, ensuring compliance with the National Framework for NHS Continuing Healthcare and Funded-Nursing Care policies and procedures.
Main duties of the job
As an Associate Practitioner, you will have completed a Level 5 Continuing Healthcare Associate Practitioner Apprenticeship, working within a dynamic team to deliver NHS Continuing Healthcare services. You should have basic knowledge and experience of working within the health sector and be assessed against the NHS England CHC Competency Framework. You will possess IT and record-keeping skills and be able to work independently and as part of a team.
The post holder should have basic knowledge of the NHS Framework for Continuing Healthcare and NHS Funded Nursing Care, along with local policies and practices related to CHC.

Detailed job description and main responsibilities
- Working within a team of Continuing Healthcare Practitioners to ensure high-quality service delivery
- Supporting workstream plans and supervising tasks effectively
- Managing own workload as delegated by the line manager
- Escalating issues promptly
- Ensuring processes comply with national and local policies
- Maintaining accurate records
- Participating in team meetings and supervision
- Supporting the team in achieving objectives
- Communicating risks, issues, and dependencies
- Staying updated on legislation and policies
- Building relationships with stakeholders and care providers

- Completing mandatory training
