Occupational Therapist – Recovery Hubs (A Council in Leeds)
Contract: Locum | Full-time
A council in Leeds is seeking an experienced Occupational Therapist to join the Recovery Hubs service, working as part of a multidisciplinary team supporting adults to regain independence and return safely to their own homes.
The Recovery Hubs consist of Occupational Therapists working alongside Physiotherapists, Nurses, and support staff to deliver a rehabilitation-focused, 24‑hour therapeutic approach aimed at promoting independence and positive outcomes for service users.
Main Duties
* Complete holistic Occupational Therapy assessments for adults with a range of needs within a reablement/rehabilitation setting
* Develop and implement goal-led, strength-based outcome plans to support independence and recovery
* Work closely with support workers and wider MDT colleagues to ensure coordinated delivery of interventions and progress reviews
* Lead on discharge planning for service users returning to their own homes
* Apply moving and handling expertise, including risk assessment and identification of safe, person‑centred solutions
* Promote single‑handed care principles and reduced dependency where appropriate
* Contribute to a 24‑hour therapeutic, rehabilitation‑focused model of care within the Recovery Hubs
* Ensure all interventions are delivered in line with council policies, statutory requirements, and professional standards
Requirements
* HCPC Registered Occupational Therapist
* Experience within community, reablement, or rehabilitation settings
* Strong assessment skills with a strengths‑based, person‑centred approach
* Confident in moving and handling risk assessment and intervention planning
* Experience working within multidisciplinary teams
* Ability to manage complex caseloads and support timely discharge planning
