The Bromley Health Visiting service is looking for enthusiastic and versatile Health Visitors who are keen to develop leadership skills and shape the future of Public Health Nursing in Bromley. Bromley 0-19 is a caring and progressive service that provides support to families with children aged zero to 19 years. Applicants would be responsible for managing a caseload of families, identifying need, providing support, and offering early intervention packages where needs are identified.

Applicants would be responsible for adopting a Think Family approach to safeguarding, with the aim of keeping children safe and well. Staff are expected to utilise their embedded skills and knowledge, as well as their clinical judgement, to implement evidence-based care planning and support packages to families. Bromley is invested in the development of Health Visiting, underpinning our Public Health Nursing strategy and training programme for all roles within the PHN teams. Current vacancies are within the Central Bromley and Orpington teams.

Main duties of the job

The main duties of the Health Visitor role are outlined in the Job specification and description. Overview of the main responsibilities include:

* Working as part of an integrated team and leading the delivery of the Healthy Child Programme
* Completing HCP mandated reviews, packages of care and safeguarding
* Adopting a Think Family approach when working with families
* Identifying health needs through an ecological assessment and, in partnership with families, agreeing personalised plans to address identified needs
* Delegating aspects of care to the Community Nursery Nurses and Staff Nurses within the skill mix team, where safe and appropriate
* Engaging in and participating in service improvement
* Continuously developing your own skills and those of others
* Working in partnership with clients and other partners, assessing healthcare needs to develop and evaluate programmes to address population health needs
* Collaborating with other healthcare and service providers to ensure seamless delivery of services for clients and communities

We currently have full and part-time opportunities ranging from a minimum of 22.5 hours to a maximum of 37.5 hours per week. This role is community-based, with duties primarily in team bases, community settings or client homes.

Detailed job description and main responsibilities

* Support collecting data to inform the performance management framework for the service, ensuring timely and accurate data collection via EMIS
* Liaise with the School Nursing Service with regard to vulnerable children, their families and children with special needs
* Participate in regular clinical and safeguarding supervision
* Attend and participate in team meetings and 1-1 management supervision
* Plan and record individual programmes of care for children and families
* Assist in identifying local health needs and ensure optimal use of resources
* Delegate work to Community Nursery Nurses within the skill mix and assist in training
* Participate in annual appraisal of work with objectives set
* Undertake mandatory and service-specific training as required
* Maintain own professional development and clinical competence
* Identify areas within the service that do not meet Clinical Governance quality standards
* Participate in Clinical Governance actions to achieve service quality
* Participate in quality improvement activities including clinical audits


Person specification


Qualifications

* Registered Nursing or Midwifery Certificate/Diploma/Degree or equivalent
* Qualified Nurse Prescriber


Professional Registration

* NMC registration as Nurse or Midwife and Specialist Community Public Health Nurse (Health Visiting)
* Experience in supervising/assessing students or holds a mentorship qualification


Training

* Evidence of Continuing Professional Development / professional updating of skills
* Evidence of further professional learning
* Knowledge of current research pertaining to Specialist Community Public Health Nursing


Skills

* Current knowledge and understanding of reports/legislation affecting community nursing including current PHE directives
* Teaching experience
* Knowledge of the public health role of contemporary health visiting
* Experience of safeguarding issues
* Evidence-based practice
* UK Driving Licence and use of a car for work purposes
* Experience in data analysis / assessing service performance against KPIs
* Experience in completing audits


Experience

* Current community experience
* Experience of working across organisational boundaries within health and social care
* Experience of working in multi-disciplinary teams
* Robust working knowledge of the Healthy Child Programme
* Clinical supervisor experience
* Management / leadership experience

Salary is determined by NHS experience and current banding/pay point. External applicants begin at the entry point of the salary scale in line with NHS terms and conditions.
