Dementia Matron

Fixed‑term secondment, 12 months. Part‑time, 15 hours per week. Closing date 28 May 2026.
Main duties of the job
* Lead a multidisciplinary team and manage a caseload of complex patients living with dementia, long‑term conditions, and/or frailty, at risk of deterioration.
* Identify patients via case‑finding or referrals from primary and acute care teams.
* Apply a case‑management approach to support patients, carers and families to live well with dementia, preventing avoidable health deterioration and hospital admissions.
* Gather and interpret complex clinical data for comprehensive health and social care needs assessment, facilitating multidisciplinary and partnership assessments.
* Implement advanced assessment and planning to deliver evidence‑based case‑management programmes.
* Oversee and audit the Dementia Matron role within Complex Care at Home to ensure adherence to the pathway across localities.
Job responsibilities
* Provide specialist knowledge and support to team members; advise on non‑complex mental health issues.
* Collaborate with patient, family and carers using health coaching / motivational interviewing to enable self‑management.
* Undertake holistic assessment and care planning for identified patients, ensuring health gains and maximising independence.
* Ensure patients with cognitive impairment have access to GHC specialist memory assessment service.
* Work autonomously as a mental health specialist linking primary, secondary and territory services to promote equitable treatment and transition to appropriate pathways.
* Act as an autonomous practitioner providing supervision and management support for team members.
Person Specification
Qualifications
* Registered Mental Health Nurse (RMN).
* Facilitating Learning and Assessment in Practice qualification (FLAP) or equivalent.
* Physical Assessment and Clinical Reasoning (PACR) qualification or willingness to undertake.
* Completed or willing to work towards the Dementia Lead course facilitated by GHC.
Experience
* Several years supporting people with cognitive impairment and physical health conditions.
* Significant post‑registration experience across a variety of settings, including community.
* Experience managing a complex clinical caseload.
* Experience with case management models, personalised goal setting, care planning, self‑management plans and escalation plans.
* Ability to chair meetings related to service delivery or case management.
* Experience managing difficult situations requiring advanced communication and negotiation skills.
* Evidence of advanced comprehensive assessment, including interpreting information, performing clinical interventions, diagnostics and analysing results.
* Post‑registration qualifications/experience at degree level or higher in managing complex long‑term conditions and frailty.

£49,387 to £56,515 a year (pro rata).
