In this role, you will: Perform comprehensive patient assessments Create and implement personalised care plans Deliver evidence-based treatments to optimise patient outcomes Collaborate with multidisciplinary teams to provide holistic care Maintain accurate patient records Requirements & Qualifications: To be successful in this role, you will need: A current and valid HCPC registration Experience working with neurological conditions (essential) Experience with both adult and paediatric patients (desirable) Exposure to or understanding of learning disabilities (desirable) Strong communication and interpersonal skills The ability to work independently in a community setting A commitment to delivering high-quality, person-centred care Working Hours & Contract Duration: This flexible position requires availability from Monday to Friday, with some flexibility based on caseload and location.
