We are seeking motivated and compassionate Community Health Practitioners (Band 4) to join our dynamic 0-19 Public Health Nursing team. This is an exciting opportunity to support children, young people and families to improve their health and well‑being, working alongside a skill mix team in a rewarding and friendly environment.

You will play a key role in delivering high-quality, person‑centred care within community settings, promoting independence, prevention and early intervention.

We have positions available in Redditch and Worcester as follows:

* 5-19 22.5 hours Redditch
* 0-5 2 x 22.5 hours Worcester

Please note the Trust will not routinely provide sponsorship to work for this role. Employment on a time limited visa doesn’t guarantee sponsorship at point of expiry.


Main duties of the job

We are a 0-19 Service with staff aligned to either 0-5 (Health Visiting) or 5‑19 (School Nursing) skill mix teams, while contributing to cross‑service delivery where required. You will be using the Solihull Approach to support CYPF and Outcomes Star to monitor and evaluate interventions.

The role involves delivering universal and targeted interventions and follow ups, providing evidence‑based support to children, young people and families in various community settings.

Some duties such as facilitating well‑child clinics, responding to Telephone Advisory Service (TAS) calls and reception year health needs assessment follow ups span across both teams.

Our 0‑5 Community Health Practitioners (CHPs) are responsible for undertaking universal mandated development reviews, introduction to solids sessions and targeted reviews, under the direction of Health Visitors.

Our 5‑19 CHPs work under the direction of School Nurses undertaking targeted follow up work from reception, Year 6 and Year 9 Health Needs Assessments and from referrals into the service.

Additional Criteria

* Ability to meet the travel requirements of the role
* Willingness to travel across the county when required

* Willingness to undertake additional training to enhance knowledge and skills.


Skills & Abilities

* Engaging with children, young people, and families in a sensitive and empathetic manner.
* Ability to communicate clearly and effectively in English in both verbal and written formats
* Capable of managing a range of priorities and planning own workload effectively.
* Awareness of own limitations.
* Ability to praise and be supportive to others
* Positive attitude and able to act on feedback.
* Ability to guide and support junior staff, students, or volunteers in health promotion and clinical tasks.
* Experience in escalating complex cases appropriately.


Experience

* Significant experience working with children, young people and families in a health, social care, or educational setting.
* Experience delivering health promotion activities or interventions, including developmental screening and emotional well‑being support
* Experience working within diverse community settings, such as schools, early years settings, or family support services.
* Practical experience identifying and escalating safeguarding concerns in line with organisational procedures.
* Proven ability to work collaboratively with multi-disciplinary teams and external agencies to deliver integrated care.
* Experience providing advice and guidance to families, maintaining professionalism in sensitive or challenging situations.
* Experience of working within the NHS
* Experience of working with children, young people and families.
* Experience of working within the Community.
* Experience of organising and delivering group sessions/workshops or parenting support programs.


Qualifications

* NVQ Level 3 in Children’s Care, Learning, and Development or equivalent qualification grades A-C or equivalent, including English and Maths.
* Care Certificate standards within 12 weeks of commencing employment if not already held.
* Safeguarding training and understanding of safeguarding policies and procedures relevant to children and young people.
* Proficiency in using basic Microsoft Office, email (Outlook), and electronic health records for data entry and record management
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.


Knowledge

* Knowledge of the Healthy Child Programme
* Knowledge of child developmental milestones and common health challenges across the 0-19 age range.
* Understanding of public health issues relating to children and their families.
* Understanding of the importance of teamwork
* Clear understanding of safeguarding responsibilities and confidence in identifying risk.
* Understanding of specific tools such as ASQ developments or school health questionnaires.
