Main Responsibilities To accurately monitor and document the care for the children and young people and ensure that the care provided adheres to individual care plans. Ensure that the children and young people’s daily routines and individualised needs are respected at all times. To be responsible for monitoring, reporting and recording any changes in the condition of the children or young people being cared for, including health, emotional and social care needs. To be competent in Derian at Home’s defined core clinical competencies and individual care needs as a Clinical Support Worker within the community setting.
