The Intermediate Care Service offers care and therapy support to patients in the Cambridgeshire and Peterborough health system following an acute hospital admission, to step down from community hospital or prevent admission. The service runs seven days a week 0700 – 2200. The vision of the service is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital as part of the HomeFirst model.
Key Responsibilities
* Provide leadership and management to the band 4 assistant care coordinators and band 2 and 3 integrated care workers working in the Intermediate Care Locality.
* Act as key enabler for collaborative working between care coordination, integrated care workers, and therapy and nursing staff.
* Undertake appraisals, supervision, training and development of bands 2, 3 and 4 staff.
* Assist the Senior Care Coordinator and Intermediate Care Team Lead to monitor and report on quality, productivity and patient flow in line with internal KPIs.
* Visit patients in the community where required to ensure quality, support patient flow and manage any issues relating to the delivery of care.
* Handle telephone complaints, enquiries and other communications with a high degree of tact, diplomacy and confidentiality in line with CPFT procedures and policies.
* Lead and manage the recruitment in line with processes and policies for all integrated care workers and band 4 care coordination staff.
* Supervise, develop and appraise the assistant care coordination staff and integrated care workers as required.
Person Specification
Experience
* Staff management or supervision experience.
* Care coordination or equivalent case management or administrative role.
* Experience within intermediate care or discharge planning pathways.
* Successful working within multi-disciplinary teams.
* Managing workload and ability to prioritise.
* Supervision training and/or experience.
* Knowledge of safeguarding policies and procedures.
* Experience working with wider stakeholder groups.
Education/Qualifications
* Level 4 in Business Administration / A Levels or equivalent acquired through experience.
* 5 GCSEs Grade A-C or 9-4 including English.
Skills & Abilities
* Knowledge of SystmOne.
* Leadership ability; able to influence, involve and develop individuals and teams.
* Commitment to high quality care and service provision.
* Decision making and problem‑solving skills.
* Use of performance management information and understanding of performance data.
* Ability to challenge and manage unacceptable behaviour.
Physical Requirements
* Must be able to travel independently around Cambridgeshire and Peterborough area meeting required timescales.
* A full UK driving licence and access to a vehicle is essential.
