Job Summary
Are you a Band 6 or an experienced Band 5 looking for an exciting new opportunity? We are seeking a skilled, motivated, and dynamic nurse with a passion for high‑quality care and service development.
You will manage your own clinical caseload of community patients, support HCA/rehabilitation assistants with training, supervision, and joint sessions. The Rehabilitation And Frailty Team (RAFT) provides daily input for up to 21 days in the patient’s own home. The service hours are 8 am – 8 pm with a care element added 7 am – 10 pm, 365 days a year. You will work occasional weekends and bank holidays on a rota basis. Car driver essential.
Main Duties
* Continuously prioritise your caseload to be responsive to changing daily needs of our service users.
* Communicate closely with other service providers to avoid admission where it is safe to do so and to provide onward referrals to other community services including health and social care.
* Work in a diverse setting, seeing a wide variety of patients: neurological, orthopaedic, falls, frail, end of life, admission avoidance, long‑term conditions, and musculoskeletal, gaining broad experience across different specialities.
* Be part of a friendly and approachable team that works well together and is well supported by senior management.
About Us
Probationary period applies. Internal applicants are exempt unless they are currently in a probationary period or bank member of staff. NELFT places a great deal of importance on new starters being properly welcomed and inducted. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks, including mandatory training, systems training, and equipment allocation. The induction will be held at our head office in Rainham, Essex.
High‑Cost Area Supplement – Fringe: This post also attracts payment for a 5 % High‑Cost Area Supplement (minimum payment of £1,303 and a maximum payment of £2,198).

Job Responsibilities
The post holder will be a Registered Nurse (RN12‑NMC) with significant intermediate and community care experience at Band 5/6. They will be responsible for leading nursing care for a designated caseload, professionally and legally accountable for all aspects of care relating to clinical areas. The post holder will assess, manage, and deliver care to meet the complex needs of patients requiring intermediate and primary care services. They will manage and support peers, working independently and as an effective team member along with pre‑registration students. The role requires working up to seven days a week.
