Overview
An opportunity has arisen for a 12 month fixed term contract within the heart failure team ending 14 March 2027 at Oxford University Hospitals NHS Trust. Covering the John Radcliffe from Monday to Friday. This is a great opportunity to apply your knowledge and expertise in cardiology/heart failure within a service that supports regular team meetings, clinical support and a strong culture of learning.
The successful applicant will join the existing Heart Failure team to:
Responsibilities
* Assess all patients admitted with heart failure.
* Provide information and advice to patients and families/carers whilst in hospital.
* Liaise with primary care clinicians, particularly community-based Heart Failure nurses in Oxfordshire and surrounding counties, to maximise continuity and quality of care.
* Undertake necessary data entry and audit requirements of the Trust to monitor treatments and quality of care.
* Contribute to the professional development of the heart failure service at a local level, providing nursing expertise for patients, families and carers.
* Assess patients admitted with confirmed or suspected heart failure and collaborate with the MDT to advise on investigations and management plans.
* Provide information, education and support to patients and family/carers, and contribute to audit processes.
* Act as a resource for nursing colleagues, contributing to educational activities to develop effective, evidence-based multidisciplinary practice.
* Provide details of informal visits and contact arrangements as required.
Qualifications and experience
* Registered nurse with evidence of relevant continuing professional development, with excellent verbal and written communication skills.
* Demonstrates keen interest in heart failure nursing with experience across the spectrum of chronic heart failure, from diagnosis to end of life care, including acute decompensations.
* Promotes and ensures the delivery of high-quality, multidisciplinary, evidence-based care throughout the patient pathway for patients and their families.
* Able to put patients at the heart of care and recognise different needs by encouraging a spirit of support, integrity, respect and teamwork.
* Shows pride in the quality of care provided and learning from successes and setbacks.
* Able to deliver a high standard of care and strive to improve through change and innovation.
* Able to proactively support colleagues in the delivery of care aligned with Trust values and behaviours.
* Registered Nurse (Part 1, 8, 12 or 15 of the register depending on area of practice) or National certification in area of practice.
* Evidence of ongoing professional development through maintenance of an ongoing NMC revalidation.
* Solid acute care registered nurse experience.
* Evidence of achievement in an NMC-approved Mentorship qualification.
* Proven clinical knowledge linked to the area of specialty with excellence in care delivery.
* Proven knowledge of safety issues through clinical governance and risk management.
* Knowledge of current nursing challenges and issues relating to the area of practice.
Desirable criteria
* UK recognised Degree in Nursing relevant to the post; postgraduate qualification related to an area of practice.
* Articulate and knowledgeable about skills needed to deliver high standard of care in the area of practice.
* Able to work under own initiative within boundaries of the role.
* Demonstrates awareness of the importance of teamwork.
* Able to document observations, results, decisions and actions effectively in patient notes and communicate these to appropriate MDT members.
* Able to communicate effectively with patients/relatives and carers and all members of the MDT.
* Able to contribute to the education of learners and non-registered/less experienced staff.
* Demonstrates ability to manage, direct and evaluate nursing care for a defined caseload of patients.
* Competent in central venous access management; excellent communication skills.
* Able to prioritise own workload and that of others as appropriate.
* Demonstrates ability to maintain and contribute to standards of practice and decision making in conjunction with team leaders/ward managers.
* Acts as a role model and leads by example to reinforce the Trust's values and behaviours.
* Able to supervise non-registered and less experienced staff to ensure effective care delivery.
* Able to escalate concerns to reduce risk and promote patient safety.
* Experience of managing clinical incidents and complaints; substantive management experience of a specific clinical area.
* Understanding of staffing and roster management, administration, and current issues related to area of practice.
* Able to keep up to date with professional issues influencing nursing practice and research.
* Able to identify learning needs and maintain ongoing professional development.
* Able to lead the development, implementation, monitoring and evaluation of policies/protocols/guidelines relevant to the area of practice.
* Demonstrates the ability to question and challenge nursing practice constructively to continually evaluate and improve patient care.
* Previous evidence of leadership within practice and the multidisciplinary team.
* Demonstrates motivation and ability to articulate reasons for wanting to work in the specific practice area; self-aware and receptive to constructive criticism; resilient under pressure; professional appearance and good self-presentation.
* Able to undertake specific procedures involving physical skills commensurate with the practice area.
* You must have appropriate UK professional registration.
