We are looking for an enthusiastic, forward-thinking team member to join the Cardiac Rehab team based at St Richards Hospital, Chichester at University Hospitals Sussex NHS Foundation Trust. Your role will include; the assessment and treatment of complex cardiac conditions, seeing patients in out-patient clinics and community exercise venues and running exercise groups. You will have very good Cardiac Rehab experience and be committed to supporting the patient to manage their condition. Excellent communication skills are a must, as well as an interest and proven ability in the supervision and clinical support of others. You will also have experience in working as part of a team.
Main duties of the job
To provide assessment and treatment for a range of cardiac patients referred to the cardiac rehabilitation service, in cross-site & community settings, in accordance with the BACPR guidelines and national/local health and safety standards.
To work in collaboration with the wider MDT and health professionals in primary care.
To supervise, teach and appraise volunteers and students
To Support the Unregistered band 5 Cardiac Rehabilitation Associate practitioner.
To participate in CPD, IST, audit, evidence based projects and service development projects

Detailed job description and main responsibilities
Communication
To maintain high levels of communication with patients, carers, other health professionals, and stake holders.
To educate patients, carers and relevant MDT staff with regards to cardiac rehabilitation risk factor reduction e.g. relaxation techniques, healthy diet and exercise.
Attend and feedback in team discussion / work including MDT and agencies.
Demonstrate good communication both informally and formally.
Employ a range of non-verbal communication skills that are non-judgmental and appropriate to the situation.
Recognise potential area of conflict or difficult conversations, know when to resolve and which channels to refer to if outside own scope, liaising with manager as necessary.
Keep accurate clinical records to agreed standards.
To establish and maintain effective communications with all members of the multi-disciplinary patient team and with other wards/departments within the hospital during the patients journey. E.g. daily handover, MDT meetings.
Service Delivery and Improvement
To manage a delegated patient caseload as referred by the Unregistered Band 5 Asso ciate Practitioner for Cardiac Rehabilitation and treat according to treatment protocols, being able to make alterations when required by assessment of patient progression. To prioritise and manage clinical work and non-clinical activities in accordance with departmental requirements.
Participate in relevant service development e.g. audit, service evaluation, user feedback and data collection.
Deliver safe and effective group sessions, adapting, monitoring and progressing patient treatment / exercise as appropriate and give advice regarding on-going exercise between sessions and at discharge.
Use initiative and problem-solving skills to resolve queries and problems, recognising when to escalate to senior staff.
To be responsible for issuing and recording outpatient appointments, to closely monitor the rehabilitation booking system, waiting list etc. and liaise with the physiotherapist and wider team when required .
To be responsible for the setting up and safe use of physiotherapy gym equipment.
To ensure treatment areas and equipment storage areas are kept to a tidy and hygienic standard.
To ensure safety and cleanliness of equipment in line with department policies.
To carry out administrative tasks, e.g. telephone calls, ordering equipment, monitoring stock and delegating tasks as required to the office volunteer.
People Management and Development
Supervise junior members of the team, and volunteers in group sessions.
Take an active role in teaching of team and volunteers as appropriate .
Ensure new and existing volunteers are competent in the use of gym equipment, reviewing annually and maintaining an up-to-date register.
Patient Care Delivery
Take responsibility for entire episodes of care for defined groups within a specialism, in which competence has been demonstrated, independently and/or with infrequent support and supervision.
To independently carry out comprehensive and appropriate assessments, treatments, including exercise prescription of patients with a variety of conditions and possibly complex presentations.
To recognise when a patient is progressing or regressing and make adaptions to the management Plan accordingly and recognise when to escalate to senior staff.
To be able to recognise when a patient has become unwell or has acutely concerning signs of ill health and be able to arrange the medical assistance required with the appropriate degree of urgency, escalate to senior staff and document appropriately e.g. Datix.
To anticipate and facilitate the discharge of patients to suitable on-going exercise programmes within the set protocol.
To be responsible for collating and entering statistical data onto the rehabilitation database.
To be involved in feeding back statistics and patient information to the MDT.
Learning and Development
Attend mandatory training updates as required.
Undertake training as necessary in line with the development of the post and as agreed with the line manager as part of the personal development process.
Achieve and demonstrate agreed standards of personal and professional development within agreed timescales.
Identify own learning needs and jointly plan training requirements with your line manager
Participate in the Trust’s appraisal process to discuss how your role will help deliver the best possible care to our patients and help to deliver any changes in service.
Person specification
Experience
* Good basic level of education.
* Knowledge of BACPR guidelines and Standards
* Relevant courses / training in specific speciality / to include Heart Failure and monitoring BACPR courses
* Evidence of relevant clinical experience in specialism, working as an autonomous practitioner at Band 4 level in an NHS Cardiac Rehab setting.
Skills
* Manage and co-ordinate ward work / clinics/ group sessions.
* Recognising when to escalate unwell patients, queries and other issues to Team lead/ nursing staff as appropriate
* Use initiative and problem-solving skills to adapt / alter exercise prescription to accommodate various co-morbidities.
