Job Summary
You will be responsible for providing a highly specialised pharmacy resource writing discharge letters, including prescribing of discharge medications, for patients being discharged from the Trust.
Main Duties of the Job
* Be a pharmacist prescriber for discharge prescriptions on the wards
* Provide, deliver and develop the pharmacy discharge service in accordance with agreed objectives
* Co‑ordinate the day to day activities for the pharmacy discharge service
* Support the Clinical Pharmacy Services manager in developing the clinical pharmacy service to the specialities in line with the needs of Stockport NHS Foundation Trust
* Attend consultant ward rounds and white board rounds as appropriate
