The role presents a fantastic opportunity to shape pharmacy services within the acute and unscheduled care teams, allowing the successful candidate to be at the heart of a fast-paced, high-impact care environment by supporting some of the most vulnerable and complex patients within our vibrant community. The ideal candidate will be passionate for the development of a new service through enhancement of patient pathways, medication safety and improvements in medicines reconciliation. Main duties of the job In this role, you will: Play a key role in developing and embedding our Emergency Department (ED) pharmacy services Drive improvements in medicines reconciliation, discharge pathways and patient experience Work closely within a multidisciplinary team to prioritise and support high-risk patients Collaborate with clinical teams to ensure patient safety and reduce prescribing errors Contribute to audit, quality improvement and innovation projects Work across multiple departments, supporting the ED as well as our acute medical and surgical assessment units, same day emergency care service and admissions wards About us Bradford Teaching Hospitals provides acute services to a diverse population across the City of Bradford and surrounding areas from our two large hospitals and three intermediate care sites.
