Housing Support Gateway Referral Officer


About the Role

Housing Support Referral Gateway Officer G09 £38,220 - £40,777 per annum 37 hours per week.

The post holder will be responsible for the appropriate delivery of Housing Support services for people who are homeless, at risk of becoming homeless or who have community safety/housing support needs across the whole of Wrexham County Borough, ensuring placements are prioritised, risk assessed and appropriate.

The post holder will need to enhance and further develop partnership working with the Housing Teams, Support Services and Housing Support providers in accepting, prioritising and co‑ordinating appropriate referrals that support people with complex and non‑complex needs in a way that enables them to maximise their independence, be able to access opportunities relevant to their needs, assessing and reviewing services and recommending changes to ensure provision is the most appropriate. They will need to have experience of chairing multi‑agency meetings to ensure that any placement / allocation is appropriate and sustainable.

At a strategic level the post holder will make recommendations to ensure joined‑up housing options and support services are delivered to people with a range of housing and support needs, gathering client evidence of service delivery that will identify gaps or duplication in service provision and ensure that these are signposted and addressed by providing the Housing Support and Development Grant Team with information which will inform service planning processes. The post will be critical to ensuring that agency work supports the strategic aims of the Local Authority, Housing Options, Housing Tenancies and the Housing Support Programmes.
