Job overview
The post holder will work in the urgent care community treatment team to help prevent inappropriate hospital admissions. Responsibilities include exercising advanced clinical expertise, conducting urgent health assessments, providing and monitoring short-term treatment plans, and ensuring safe discharge plans through collaboration with primary, secondary, and social care providers. The role requires autonomous practice in assessing, diagnosing, and treating patients presenting with health crises, working with a multidisciplinary team to deliver high-quality care.
Main duties of the job
1. Manage a specialized caseload within the community using assessment, planning, implementation, and evaluation.
2. Provide professional and clinical support to staff within CTT and collaborative services.
3. Participate in developing a high-quality clinical governance framework.
4. Lead the day-to-day management of CTT, coordinating workload and team development.
5. Implement service developments impacting Community Services.
6. Establish effective communication across disciplines and services.
7. Assess, plan, implement, review, and evaluate care autonomously, including complex assessments.
8. Act as a role model, resource, and mentor, promoting excellence and standards of care, including clinical supervision and peer review.
9. Participate in service requirements such as auditing and supervising junior staff and students.
Working for our organisation
This post is subject to a probationary period, exempt for internal applicants unless specified. It attracts a High-Cost Area Supplement of 15% of the basic salary, with a minimum of £4,551 and a maximum of £5,735.

Detailed job description and main responsibilities
This is a community-based service assessing patients at home, focusing on physical health needs. The service operates 365 days a year from 8am to 10pm, with various shifts. It includes an emergency response vehicle operated in collaboration with London Ambulance Service, with shifts of 12 hours. Training will be provided.

Education and training
* First level Registered Nurse, registered with the NMC, BSc (Hons)
* Physical Clinical Assessment Skills Course
* Practice educator or equivalent qualification
* Non-Medical Prescriber (NMP)
* Postgraduate diploma in Urology nursing
* Management training or equivalent experience
Experience, skills, and knowledge
* Experience as a RN/Paramedic in a Band 6 or equivalent role, working autonomously for at least 1 year
* Experience in advanced physical assessment and examination skills
