Job overview

Clinical Caseload Holder - Band 7 - Beckenham area

Full time - 37.5 hours per week

Applicants must hold the DN SPQ qualification.

The Clinical Caseload Holder is responsible for the overall operational and day to day management of the community nursing team within their identified caseload.

They will be responsible for managing the team to ensure that quality of care, financial and performance targets are met and sustained. It is expected that the postholder will act as a clinical expert in relation to District Nursing, providing clinical supervision and line management supervision to their team.

They will work hand in hand with the PCN Clinical Neighbourhood Manager, and Clinical Caseload Holders across a range of issues and areas to ensure there is consistency and quality nursing care across the community teams and neighbourhoods.

The expectation is that the Clinical Caseload Holder will work with the Safer Care directorate to provide evidence based, in depth clinical and professional knowledge and expertise for the nursing element of the service.

This role is to cover the Beckenham area.


Main duties of the job

There is an expectation that the post holder will work flexibly across the week during the core District Nursing hours of 8.15am - 18.45pm and participate in working 1 in 4 weekends inclusive of bank holidays involving patient visits and also the Nurse of the Day.

The Clinical Caseload Holder will be responsible and accountable for a defined caseload of patients, and will be a key member of the Community Team having clinical responsibility for a defined group of patients, providing robust clinical services for patients, working closely with allocated GP practices and with staff from other professions, putting the patient at the centre of their care.

Professional Registration

* NMC Registered Nurse - Adults


Information Technology

* Ability to use and interrogate software to support client care e.g. dashboards, malinko, EMIS, electronic records, email
* Knowledge of EMIS
* Knowledge of Malinko


Specific Skills

* Ability to lead a team and manage/develop staff
* Management and investigation of complaints and incidents
* Able to articulate well both verbally and written. Able to provide and receive complex or sensitive information
* Able to analyse and make decisions in relation to risk and concluding patient/client care in complex cases
* Demonstrates an awareness of local and DOH legislation and its effects on service both nationally and locally
* UK driver’s licence with access to own car with clean driving licence
* Ability to work all required shifts including weekends and bank holidays
* Root cause analysis


Experience

* Significant Community Nursing experience
* Experience of working in a multi-disciplinary team
* Clinical Supervision experience
* Demonstrates evidence of involvement in development of protocols and service changes as a result of clinical audit
* HR management of staff – including one to one’s and appraisals
* Involvement in change/project management
* Performance management of staff, implementing personal improvement plans

* Evidence of Continuing Professional Development relevant to District Nursing

Qualifications

* Degree level education
* RN (Adult Branch)
* District Nurse Qualification (or relevant experience for the first change programme)
* Prescribing Module (V100/V300)
* NMC Registered Nurse - Adults
