Overview
The purpose of the role is to work as part of the QOF team and support the clinicians at Tyntesfield Medical Group to achieve all QOF targets annually.
Responsibilities
* Manage our chronic disease register and ensure patients are called for reviews in a timely manner.
* Assist with achieving Quality Outcome Framework (QOF) targets; maintain data awareness and accuracy.
* Promote attendance at Health Promotion clinics and carry out basic searches; working knowledge of EMIS is desirable.
* Provide excellent customer service and be a team player in a patient-centred environment.
Qualifications and experience
* Good understanding of Quality Outcome Framework (QOF) targets and working knowledge of EMIS is desirable.
* Previous NHS experience preferred, especially in promoting attendance at Health Promotion clinics and ensuring patients are called for reviews in a timely manner.
* Strong data awareness and personal skills.
