Overview
An exciting opportunity has arisen for an Advanced Clinical Practitioner to join the Banff and Buchan cluster. The role is a diverse and autonomous clinical position delivering enhanced clinical assessment and treatment to meet patient needs in the community setting. You will support medical staff and other health care professionals to provide optimum service delivery and will work in collaboration with independent practices to deliver high-quality care throughout clusters, undertaking acute on-the-day triage and home visits when required. The post can involve work across GP surgeries, community hospitals and patients’ homes.
We are looking for a suitably qualified Advanced Nurse Practitioner/Advanced Clinical Practitioner with evidence of advancing practice and a sound understanding of conditions that may require hospital admission or virtual community ward management. You should recognise and initiate management of deteriorating patients and have experience and clinical knowledge for managing patients at home as part of an MDT. Applicants should be proactive in expediting the patient journey and developing the role of the Advanced Clinical Practitioner within Aberdeenshire as part of a multidisciplinary team.
Educational requirements align with The Advanced Care Academy framework, including a portfolio, with completion mandatory within an agreed time frame. Hours of work are flexible to meet service needs. A valid driving licence and access to a vehicle are essential, with willingness to travel within the Banff and Buchan cluster.
Responsibilities
* Deliver enhanced clinical assessment and treatment in community settings.
* Provide acute on-the-day triage and undertake home visits when required.
* Support medical staff and other health care professionals to ensure optimum service delivery.
* Collaborate with independent practices to maintain a high level of care across clusters.
* Manage care across multiple clinical areas including GP surgeries, community hospitals, and patients’ homes.
Qualifications
* Qualified Advanced Nurse Practitioner or Advanced Clinical Practitioner with evidence of advancing practice.
* Sound understanding of conditions that may require hospital admission or virtual community ward management.
* Experience and knowledge in managing patients in their own homes as part of a multidisciplinary team.
* Proactive in expediting the patient journey and developing the ACP role within Aberdeenshire.
* Educational framework aligned with The Advanced Care Academy; portfolio completion within an agreed time frame.
* Flexible hours, driving licence and willingness to travel within the cluster.
