We are recruiting for an experienced Practice Nurse to join our amazing Nursing team which work across three surgeries.
You will be providing high quality care based on the needs of our patient population which includes treatment, screening, preventative care and patient education.
Main duties of the job
We welcome applications from applicants that have demonstrable experience in either asthma, COPD or diabetes reviews along with experience in chronic disease management
You will support and manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems.

Job responsibilities
ClinicalPractice
Experiencein either asthma, COPD or diabetes reviews is essential
To managepatients with chronic disease, running specific clinic sessions and maintainingrobust call and recall systems
Experiencein chronic disease management
Participatein national screening work such as cervical cytology and deliver the nationalimmunisation and vaccination schedule
Providetravel health advice and administer appropriate vaccinations
Providedirectly and assist GPs with contraceptive services
Breakbad news to patients/parents/carers and provide appropriate ongoing support tomeet identified needs. Signpost patients to relevant support networks
Actas a chaperone when required
Undertakehealth promotion to help educate patients
Select,carry out and interpret results of specific tests
Undertakeanticoagulation clinics to include interpretation of blood tests and dosing
Communication
Demonstratesensitive communication styles to ensure patients are fully informed andconsent to treatment
Communicateeffectively with patients and carers, recognising the need for alternative methodsof communication to overcome different levels of understanding, culturalbackground and preferred ways of communicating
Anticipatebarriers to communication and take action to improve communication
Estimateand maintain effective communication with individuals and groups within thepractice environment and with external stakeholders

Administration
Tomaintain accurate patient records and enter onto the system using agreedRead Snomed Codes
Ensureaccurate completion of all necessary documentation associated with patient care
Tocollect data and participate in regular Practice audit and research projects
Tocontribute to the preparation of any Practice development plans
Tomaintain own annual administration with the NMC and adhere to Code ofProfessional Conduct
Tomaintain a safe and therapeutic environment for all PHCT staff, patients,carers and co-workers from other agencies.Ensure all statutory regulations e.g. Health and Safety, COSHH etc. are implemented and adhered to
Topromote a professional approach at all times by attire and attitude to patientsand staff both in and out of the Practice
Tomaintain own continuous professional development and ensure PDDP kept upto date in line with the requirements of the practice
Attendand participate in staff and Practice meetings and in house clinical trainingsessions
Anyother reasonable duties that may be required from time to time
