We are looking for a motivated and enthusiasticNurse Practitioner to join our friendly, patient focused practice team.
Previous experience in minor illness and triage ina primary care health experience is essential. You must be able to demonstratesafe, clinical decision making for patients acting within professionalboundaries.
Applicants will have completed their NursePractitioner qualification and be confident working autonomously in primarycare, as well as under the guidance of our GPs.
The post holderwill be an experienced nurse practitioner who, acting within their professionalboundaries, will provide care for the presenting patient from initial historytaking, clinical assessment, diagnosis, treatment and evaluation of care. Theywill demonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills, for patients within the general practice.
They willdemonstrate critical thinking in the clinical decision-making process. Theywill work collaboratively with the general practice team to meet the needs ofpatients, supporting the delivery of policy and procedures, and provide leadershipas required.

Job responsibilities
Patient Triage/ Appointment Hub-assessing andprioritising patients based on the severity of their condition to determine theurgency of care needed and ensuring that the most critical cases are seen first.
Offering immediate medical advice and guidance to patientsover the phone or in person.
Identifying and escalating patients requiring immediateemergency care or specialised attention.
Clinically examine and assesspatient needs from a physiological and psychological perspective, and planclinical care accordingly by telephone triage or face to face consultations.
Assess, diagnose, plan,implement and evaluate interventions/treatments for patients with complexneeds. Proactively identify, diagnose and manage treatment plans for patientsat risk of developing a long-term condition (as appropriate)
Diagnose and manage both acuteand chronic conditions, integrating both drug- and non-drug-based treatmentmethods into a management plan
Prescribe and reviewmedication for therapeutic effectiveness, appropriate to patient needs and inaccordance with evidence-based practice and national and practice protocols,and within scope of practice
Work with patients in order tosupport compliance with and adherence to prescribed treatments. Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions
Prioritise health problems andintervene appropriately to assist the patient in complex, urgent or emergencysituations, including initiation of effective emergency care
Support patients to adopthealth promotion strategies that promote healthy lifestyles, and applyprinciples of self-care
Utilise and demonstratesensitive communication styles, to ensure patients are fully informed andconsent to treatment
