Older Adult Operational and Clinical Pathway Lead


NHS AfC: Band 8b

Site Pennine Care NHS Foundation Trust Town Ashton under Lyne Salary £66,582 – £77,368 per annum Salary period Yearly Closing 18/06/2026 23:59

Working closely with the clinical director for older adults, the post‑holder will provide operational and clinical leadership across the Older Adult Pathways on a Trust‑wide basis.

This role will provide senior oversight to ensure that care pathways are robust and reflect best practice guidance whilst supporting the Trust’s strategic ambition to provide outstanding care through quality assurance mechanisms.

The post‑holder will need an in‑depth knowledge and understanding of the national, regional and organisational context with regards to older adult clinical services.

Education / Qualifications

* Educated to masters level (or equivalent) in relevant area
* Qualified and registered RMN/Social Worker/AHP
* Post graduate training relevant to mental health / leadership / management
* Evidence of continued professional development
* Management / leadership qualification
* Other relevant clinical qualification


Experience

* Significant experience as a senior clinical practitioner in older adult mental health
* Management and leadership of mental health services
* Management of performance relating to the admission and discharge process, bed base, and reducing delays to discharge.
* Delivering service / practice innovation, development and change
* Knowledge of financial management and systems of budgetary control
* Track record of leading and motivating staff teams and of developing a performance culture
* Participation in local/GM and regional professional forums
* Experience of managing integrated teams
* Experience of multi‑agency working
* Experience of managing change, developing business cases and service plan developments
* Significant experience in managing service users with complex, challenging and dangerous presentations.
* Formal research experience
* Clinical experience in a range of mental health settings


Knowledge

* Comprehensive knowledge, understanding and application of the national Mental Health agenda, current best practice in Mental Health and the Mental Health Act.
* Ability to lead strategic planning and regarding the older adult pathway agenda.
* Resolution of conflict / professional / ethical matters
* Proven effective leadership skills
* Knowledge of governance and best value practices
* Proven planning and decision making skills alongside a demanding workload
* Ability to establish professional boundaries and resolve operational issues
* Audit and research skills


Skills and Abilities

* Knowledge and application of effective time management and prioritisation skills
* Ability to maintain effective working relationships and good communication with relevant stakeholders
* Ability to motivate staff to achieve goals
* Ability to recognise and work through resistance to changeAbility to prioritise workload and achieve objectives within agreed deadlines
* Ability to lead by motivating, supporting and encouraging staff to develop personally and professionally
* Excellent written and verbal communication skills
* Ability to reflect on and critically appraise own performance
* Well-developed Information Technology skills


Work Related Circumstances

* Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
