Overview
We are looking for an experienced, dedicated hardworking Mental Health Pharmacist to lead our clinical pharmacy team providing services to rehabilitation for complex psychosis services working across South West London on multiple sites. You will be joining an innovative award-winning pharmacy team who are forward thinking and are at the forefront of using technology to support the medicines optimisation agenda. The role will include working as a non-medical prescriber and holding a caseload of patients on the complex psychosis pathway alongside providing highly specialist advice to clinical staff, service users and carers. You will deliver and support the complex care pathway, including Riverside, Bridge, Phoenix, Burntwood Villas, community teams, and primary care and provide second opinion support and oversight of medicines optimisation across inpatient and community rehabilitation services. You will be part of the ongoing service development of the community rehabilitation team and rehab expansion project. You will also be an integral member of the pharmacy senior leadership team and lead the on-going development of pharmacy services within the rehabilitation services line as well as supporting Trust and Integrated Care System level medicines optimisation initiatives.
* Manage the delivery of a comprehensive medicines management service to the SWLStG Rehabilitation services to meet the evolving needs of the NHS.
* Provide input into policy and procedures affecting the use of pharmaceuticals at both local and trust wide levels.
* Lead Mental Health Pharmacy Services for the specified service area(s) with the Trust in accordance with the priorities set by the Chief and Deputy Chief Pharmacist, Rehabilitation Consultant, Rehabilitation team manager.
* Lead medicines management service developments and present the projects to committees where necessary across the specified borough/service area(s) and Trust-wide where needed.
* Ensure delivery of the Pharmacy clinical governance agenda within the specified borough(s).
* Proactively develop clinical pharmacy services in line with local and national objectives.
* Manage pharmacy staff involved in providing a service to the borough/service area(s).
* Evaluate the contributions of the specified borough/service area(s) Pharmacy Service.
* Provide safe, clinically effective and cost effective use of drugs in patients within the agreed borough/service area(s).
* Reduce risk associated with medicines use by contributing to the safe medication practice agenda, including identification and reporting of medicines related incidents.
* Critically appraise the literature and give accurate interpretation in order to improve the use of medicines.

Responsibilities
* To co-ordinate and lead in the planning, development, management, and provision of clinical services to mental health rehabilitation services within the Trust.
* To support and deliver the Medicines Optimisation strategies set by the Service Line and the Chief Pharmacist. Communicate complex medicines processes and information to prescribers, clinicians, patients, relatives, and carers.
* To lead medicines management service developments and present projects to committees where necessary across the mental health rehabilitation services and Trust-wide where needed.
* To be responsible for the coordination of the production of Trust prescribing guidance (information gathering, stakeholder meetings, research and presenting the guidance at the relevant committee).
* Demonstrate innovation and extend the boundaries of the profession.
* Review and propose changes to Trust medicine related policies and decisions.
* Promote good prescribing practice throughout the boroughs and Trust (where applicable), in accordance with the Formulary.
* Act as a clinical role model and demonstrate the ability to provide safe, clinically effective and cost efficient use of medicines for junior staff within Rehabilitation services.
* Identify and implement a vision for the delivery of pharmaceutical & clinical services to the mental health rehabilitation services.
* Proactively develop clinical services with the pharmacy team and service line in line with local and national objectives.
* Ensure the pharmaceutical service provided meets the Care Quality Commission’s and RPS medicines related standards.
* Ensure delivery of the medicines optimisation clinical governance agenda within the Service Line.
* Work collaboratively with senior pharmacy and medical staff, the multidisciplinary team and management and participate in the medicines management agenda as agreed.
* Represent Pharmacy and/or the mental health rehabilitation services at relevant meetings and conferences, local and national.
* Assist the Chief Pharmacist with the modernisation agenda for pharmacy services in partnership with acute trusts.
* Coordinate care across the mental health rehabilitation pathway, ensuring integrated medicines optimisation.
* Support rehabilitation commissioning activities through expert input and strategic advice.
* Establish links with other Mental Health Trusts to stay informed on latest developments and innovations.
* Embedded within both the pharmacy and rehabilitation teams to promote collaborative leadership.
* Attend regular rehab and pharmacy team meetings to maintain alignment and drive service improvement.
