Responsibilities
* All aspects of urgent care in a primary care setting including telephone triage, face to face consultations in primary care centres across our 10 NHS Lanarkshire Localities and home visiting.
* There is a requirement to provide PLT to all GP practices 3 times per year on a rotational basis.
* The cover is required between 1400-1800hours for 20 sessions per annum to support PLT in each of our 10 Localities across Lanarkshire.
* Successful candidates will be provided with training to facilitate use of the Adastra Patient Management System and Clinical Portal.
* You will have relevant, demonstrable experience and a track record of success in a team with colleagues in your own and other disciplines. You must be able to evidence commitment to patient-focused care, continuous professional development and life-long learning. You will be able to demonstrate learning and continuing professional development. You will have the ability to take full responsibility for independent management of patients and communicate effectively and clearly with patients and other team members.
Qualifications
* Experience and confidence in urgent care aspects; advantageous.
* You must be able to evidence commitment to patient-focused care, continuous professional development and life-long learning.
