Overview
A new, exciting opportunity has arisen for enthusiastic, organised and motivated individuals to join Cancer Services & Clinical Haematology as a Patient Navigator. The role involves working closely with clinical staff within tumour groups, providing an efficient referral service to Cancer Services, managing the flow of new patients through a single point of access and appointing appropriate appointments within pathway targets to meet cancer targets. The successful candidate will demonstrate excellent communication skills, flexibility, reliability, an organised approach to work and attention to detail to support patient flow through the service.
Responsibilities
* Request scans and results from relevant external healthcare providers.
* Make new‑patient appointments in the appropriate clinics within the required timescale, ensuring cancer targets are met.
* Talk to patients over the telephone and ensure their queries are dealt with efficiently.
* Work with other departments within the board, including the radiotherapy and chemotherapy teams.
* Perform ad‑hoc duties as requested by the clinical team.
* Work within a team of Patient Navigators and admin support, led by the clinic coordinator, to provide a comprehensive referral service.
* Communicate with consultants, medical staff and other clinical members of the team, e.g. specialist nurses.
* Communicate with patients regarding appointments and admissions at the request of the medical staff.
* Provide cover for pathway navigator/clinic coordinator as required.
* Cover administrator support posts trust‑wide as required.
* Manage the flow of patients through the single point of access service.
* Administer referral spreadsheets and maintain information management and data quality.
* Receive and make calls as required.
* Work flexibly within the team, providing support and training to colleagues.
* Supervise, train and develop other clerical staff.
* Maintain awareness of service delivery developments and contribute to the implementation of change within the service.
* Ensure efficiency of the appointment system; monitor patient flow into the system and report any concerns.
* Liaise with relevant staff from various healthcare settings reporting issues and concerns.
* Provide comprehensive administrative and clerical support, including issue logs.
