An exciting opportunity has arisen for a full time enthusiastic, vibrant Senior Staff Nurse to work in our community branch of the Mental Health Rehabilitation Service. We are based at Bodmin Hospital site in Cornwall (St Austell).
The Community Mental Health Rehabilitation team provides a service across the whole county of Cornwall, supporting people recovering from complex and enduring mental health issues. The team works alongside the inpatient facility Fettle House to provide a pathway for patients as they resume/ establish independent lives outside of the hospital setting. The team collaborates within Cornwall Foundation Trust, including carers, families and other specialists (e.g., Early Intervention Team). We assist with discharges from acute wards and work with housing providers, education and pharmacy. We take a person-centred approach with individualised dialogue and care plans. The post holder will have a key role supporting discharges from acute wards, supervising junior staff, and participating in the ongoing review and development of the rehabilitation service. Regular individual support and supervision is provided.
Main duties of the job
To be responsible for the assessment of care needs and clinical management of the rehabilitation outreach caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
To carry continuing responsibility for the development, implementation, and evaluation of programs of care, utilising the care co-ordination system/lead HCP role.
The Senior Staff Nurse is expected to provide clear clinical operational leadership to the Community Team, including staff nurses and healthcare assistants. They will demonstrate exemplary practice.
This is a community-based role that requires frequent travel to various locations throughout the county, including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
