This 12-week locum position offers a competitive hourly rate of £27 and requires immediate availability to work Monday to Friday from 9 am to 5 pm. The position is available at standard CAP rates for Band 6 roles.

What you will do
* Conduct comprehensive assessments of patients within the community, ensuring each client receives personalised and meticulous care plans.
* Liaise effectively with a multi-disciplinary team to coordinate care services, ensuring seamless and collaborative care delivery.
* Provide support and guidance to patients and their families, helping them navigate the healthcare system and access appropriate services.
* Monitor and evaluate care plans, making necessary adjustments to guarantee the highest standards of patient care.
* Document and maintain accurate records of patient interactions and care plans.
