About the Role
As a Team Manager, you will play a pivotal role in leading and supporting a multidisciplinary team comprising Social Workers, Social Care Workers, Trusted Assessors, and Business Support Officers. You will be responsible for the day-to-day operational management of the team, ensuring high-quality service delivery that facilitates safe and timely hospital discharges using the Discharge to Assess model.
You will oversee the completion of Care Act assessments and reviews, whether in discharge-to-assess placements or individuals’ homes, ensuring that interventions are strengths-based and outcome-focused.
This is a dynamic leadership role requiring excellent organisational skills, sound decision-making, and the ability to manage complexity. You will be a visible and supportive leader, providing regular supervision, coaching, and guidance to your team, while working closely with health partners and other agencies.
Key Responsibilities
1. Lead and manage a team of social care professionals to deliver high-quality, timely support in a hospital setting.
2. Ensure effective discharge planning and assessment processes aligned with the Discharge to Assess model.
3. Provide regular supervision and performance management to team members.
4. Work collaboratively with health and social care colleagues to promote integrated working.
5. Support staff to embed strengths-based approaches in their practice.
6. Contribute to service development and continuous improvement initiatives.
7. Maintain oversight of complex cases and support practitioners in decision-making.
8. Ensure compliance with statutory responsibilities and council policies.
Key requirements
9. A qualified Social Worker registered with Social Work England.
10. Proven experience in adult social care, ideally within a hospital or acute setting.
11. Strong leadership and supervisory experience with the ability to confidently prepare for and lead team meetings, as well as other key discussions/meetings as required.
12. Able to work across a 7-day rota, including one weekend every four weeks, to support a streamlined service and ensure timely hospital discharges. Candidates must be prepared to work weekends as part of the team’s commitment to responsive care.
13. To work closely with the Multidisciplinary Health & Social Care Team.
14. Excellent communication and interpersonal skills.
15. A confident decision-maker with the ability to manage competing priorities.
16. A commitment to strengths-based, person-centred practice.
17. Proficiency in IT systems – experience with Mosaic is desirable.
18. A full UK driving licence and access to transport is essential.
