We are looking for a permanent, full‑time Social Worker as part of our Urgent Care and Short Term Support team. The role will support people across Lancashire, working closely with community services to prevent hospital admissions and facilitate early discharge.
Key responsibilities include:
* Providing a single point of access to community intermediate care services, including reablement and residential rehabilitation, 7 days per week.
* Supporting hospital discharges and collaborating with multidisciplinary teams to keep people at home.
* Using the Discharge to Assess model to conduct assessments outside the hospital environment.
* Delivering holistic, face‑to‑face Care Act assessments, reviews and support planning, applying a strengths‑based approach to enable participants to achieve independent living.
