Senior Clinical Nurse Specialist – Palliative and End of Life Care (Great Western Hospital Foundation Trust)

We are excited to advertise for an additional highly motivated Senior Clinical Nurse Specialist to join the Palliative and End of Life Care Team at Great Western Hospital Foundation Trust. The team is dedicated to ensuring high-quality clinical care and positive experiences for patients, their families and carers with life‑limiting diseases requiring specialist palliative and end‑of‑life care.
The ideal candidate will have a flexible and proactive attitude. Working alongside the multi‑disciplinary team across organisational and professional boundaries, you will support the delivery of clinically effective, evidence‑based care that reflects current best practice for people with complex specialist palliative and/or end‑of‑life care needs. You will lead, inspire and empower others, influence patient care through direct management, specialist education and training to multi‑professional staff, statutory and voluntary agencies, and undertake audits and service development within palliative and end‑of‑life care.

Job Responsibilities
* Demonstrate experience of working with patients in a variety of settings, and the ability to work both autonomously and as part of a team.
* Adopt a flexible approach to meet changing needs of palliative and end‑of‑life care, responding to evolving demands of the Trust, patients and their families.
* Support the delivery of a comprehensive multi‑disciplinary service that is holistic, evidence‑based and meets individual patient needs.
* Engage in education, development and empowerment of healthcare colleagues and Trust employees interacting with patients requiring palliative care and end‑of‑life support.
Essential Qualifications
* Registered Nurse Level 1, registered with the Nursing and Midwifery Council (NMC)
* Advanced communication skills / psychological level 2 training
* Nurse Independent Prescriber or working towards it
* Advanced history taking and assessment
* Competence in discussions and decision‑making regarding Adult Treatment Escalation Plan (ReSPECT) and resuscitation
* Professional/clinical knowledge from a degree, supplemented by specialist training, experience, short courses and CPD to master's degree level or equivalent
* Evidence of ongoing continuous professional development
* Teaching & assessing qualification or equivalent experience
* Leadership qualification
Essential Experience
* Post‑registration experience in specialist palliative care
* Line management experience
* Evidence of ability to work autonomously
* Evidence of leadership experience
* Experience in change management
* Used to working under pressure with competing demands
* Significant experience at band 6 or above
* Experience in developing and training staff
* Evidence of involvement in clinical governance
Desirable Qualifications
* Varied clinical background
* Risk management and incident investigation experience
* Experience leading a specialist team
* Experience in service‑user advocacy and complaints resolution
* Project management experience
