What youll be doing
* Delivering high-quality nursing care in patients own homes.
* Supporting early hospital discharge and preventing avoidable readmissions.
* Building meaningful clinical relationships with patients and MDT partners.
* Completing IV therapy administration, wound care, venepuncture, and general nursing duties.
* Working a mix of early and late shifts across a 7-day rota.
