The post holder will be responsible for the assessment of the individuals in the community/ home/ nursing homes and hospitals to determine eligibility to receive NHS Continuing Healthcare or Funding Nursing Care. The assessor will act as a member of the multidisciplinary team to co-ordinate the process, establish eligibility and assist in identifying a suitable care package. The assessor will be responsible to ensure the review process for all NHS Funded Care is undertaken in accordance with current guidance.
Main duties of the job
The practitioner will utilise the national assessment tools and adhere to the required timescales, co-ordinate, participate and support the assessment process with the multidisciplinary team. The role will be part of the MDT and determine the individuals eligibility in line with the NHS Framework for NHS Continuing Healthcare and Funded Nursing Care. This will encompass explaining the process to patients and families; health and social care colleagues and care providers. Ensuring that the process is transparent and clearly understood by all parties.

Qualifications
* Evidence of on-going post-basic qualifications in area identified
* Highly skilled clinical nurse competent to practice within expanded role
* Educated to degree level (or equivalent/ evidence of working to)
* Evidence of commitment to lifelong academic learning.
Experience
* Excellent verbal, written and presentation skills.
* Computer literate- ability to use software programmes to maximise contribution to the post.
* Demonstrate knowledge of current legislative changes affecting the NHS and the Local Authority.
* Application of the NMC Code of Conduct and Scope of Professional Practice.
* Project management skills to produce outputs within set timescales.
