Winter Pressures Advanced Nurse Practitioner (ANP)

Are you a skilled and motivated Advanced Nurse Practitioner looking to make a real impact in primary care?

We are seeking an ANP to join our dynamic and supportive multidisciplinary team. You will work both autonomously and collaboratively to deliver high-quality, comprehensive care to a diverse patient population for a period of 6 months to cover October-March to provide additional 'Winter Pressures' appointments.

In this role, you will undertake clinical assessments, diagnosis, treatment, and management of both acute and chronic health conditions. You will focus on delivering person-centred care, tailoring treatments and support to meet the unique needs of each patient.

This role offers an exciting opportunity to work within a progressive primary care network dedicated to improving patient outcomes through compassionate, evidence-based practice.

This post is offered as a full time position over 5 days (4 days at Whickham, 1 day at Sunniside) from October 2025 - 31st March 2026.

Interviews would likely be the morning of Wednesday 22nd October.


Main duties of the job

The Advanced Nurse Practitioner will play a crucial role in delivering additional appointments during the winter season to increase capacity. As part of a multidisciplinary team, you will provide timely, high-quality clinical care through thorough assessment, diagnosis, treatment, and management of patients.

Key responsibilities include:

Performing detailed clinical assessments and physical examinations

Diagnosing and managing both acute and chronic health conditions

Independently prescribing medications within your professional scope

Ordering, interpreting, and acting on diagnostic tests and investigations

Leading reviews for long-term conditions and creating tailored care plans

Delivering patient education and promoting health and wellbeing using evidence-based approaches

Collaborating closely with other healthcare professionals to ensure coordinated and seamless patient care

This patient-facing role demands excellent clinical decision-making, strong communication skills, and a compassionate commitment to providing person-centred care in a primary care environment.


About us

Based in the Inner West of Gateshead, Whickham Practice serves 15,650 patients. We have 5 partners, 5 salaried GPs, a Paediatric ANP, and nursing team of 10, along with an experienced and supportive reception and administration team. We are a high QoF achieving practice and are an active member of our PCN.

We provide integrated services to our local population. Aiming to deliver more personalised, coordinated health and social care for people close to home.


Job responsibilities

Clinical Support

Work autonomously and collaboratively within the multidisciplinary team, following local practice guidelines and protocols.

Lead clinical assessment, diagnosis, and management of patients across a broad range of acute and chronic conditions.

Support GPs and other healthcare professionals in delivering comprehensive patient care, including preventative health and health promotion.

Attend mandatory training and professional development sessions to maintain and enhance clinical skills.

Deliver nursing care independently or in partnership with GPs and nurses, adhering to agreed protocols and professional standards.

Clinical Tasks

Perform advanced clinical assessments and physical examinations.

Conduct diagnostic tests, including ordering, interpreting, and acting on results.

Prescribe medications independently within the scope of practice.

Manage long-term condition reviews, developing personalised care plans.

Record and monitor vital signs including blood pressure, pulse, temperature, BMI, and urinalysis.

Prepare and maintain clinical areas and equipment to ensure safe and efficient patient care.

Participate in health promotion, patient education, and self-management support.

Escalate complex or urgent clinical concerns to senior clinicians promptly.

Communication

Communicate sensitively and effectively with patients, carers, and colleagues.

Adapt communication style to meet the diverse needs and understanding of patients.

Provide clear explanations and reassurance, especially when delivering difficult news.

Maintain professional and respectful communication within the team and with external partners.

Advocate for patients needs and preferences in care planning and decision-making.

Quality of Care

Practice within your competence and professional scope.

Manage clinical workload effectively while maintaining high standards of care.

Deliver care in accordance with NICE guidelines, local protocols, and evidence-based practice.

Maintain strict confidentiality and comply with data protection laws.

Person Specification


Qualifications

* Registered Nurse with current Nursing and Midwifery Council (NMC) registration
* Qualified Independent Nurse Prescriber, registered with the NMC
* Meet NMC revalidation requirements as outlined in the NMC Revalidation booklet
* Meet the standards for registered Advanced Nurse Practitioners working at an advanced level
* Qualified triage nurse


Experience

* Experience working within a primary care setting
* Proficient in advanced clinical practice skills
* Skilled in managing patients with long-term conditions, complex needs, and chronic diseases
* Experienced in nurse-led triage
* Competent in using EMIS clinical systems
* Able to work confidently within own scope of practice and recognise when to escalate or refer to GPs
* Excellent verbal and written communication skills
* Strong IT proficiency, including Microsoft Office and Outlook
* Effective time management and organisational abilities
* Able to work both independently and as part of a multidisciplinary team
* Experience of working in primary care / GP Practice / OOH
