We are looking for a Clinical admin support worker to join our Community nursing service. This post will be forlocality cover for the Sedgefield Community Nursing teams.
Main duties of the job
The Community nursing team provide both plannedand reactive care including Urgent Community Response.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

From April 9th 2025 - Healthcare assistants - You must have a minimum of two yearsof experience working within the NHS as a HCA for the Trust to considersponsorship (we will only consider sponsorship if you meet this criteria andhave a current visa expiring within 3 months).
Job responsibilities
The successful applicant will be required to workwith the community nursing, supporting the locality with processing of nursingreferrals and supporting the team with office duties and admin functions. Full training can be given.

Special Skills & Knowledge
* Good verbal and written communication skills.
* Good organisational and facilitation skills.
* Ability to work independently.
* Ability to use audit methodologies.
* Preparing excel spreadsheets and reports.
* Clinical knowledge.
* Able to assess service user needs, apply appropriate care plan and allocate the visit to the appropriate professional to deliver the care.
Experience
* Experience of using trust electronic systems such as SystmOne and telephone call handling.
Special Requirements
* Able to travel across Trust sites to meet the requirements of the post.
Qualifications
* NVQ Level 3 or equivalent in relevant area, e.g diagnostic and Therapeutic Care or Care or working towards or equivalent experience.
* Knowledge of Microsoft packages.
* An understanding of County Durham and Darlington NHS Foundation Trust, its culture, strategic direction, achievements and NHS standing.
