Newent Doctors Practice has anexciting opportunity for a practice nurse to join our expanding practice in semi-rural Newent. Newent is a small market town which is central for a Gloucester, Ross,Malvern, Ledbury, or a Tewkesbury commute.
We are looking for an experienced practice nurse but are alsowilling to train the right nurse for a long-term role.
We are offering a position of 28hours per week across four days.
Salary is negotiable depending onqualifications and experience.
Please note you will be expectedto support our enhanced access clinics and work a late shift every Thursday evening as part of your weekly contractual hours and approximately 3 Saturdaymornings a year for which you will be paid overtime.

Main duties of the job
The following are the core responsibilities of the practice nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
* To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy
* To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence based practice
* Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
* To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
* Provide routine nursing care to patients in accordance with clinic based evidence, NICE and NSF
* To diagnose and manage acute and chronic conditions as required
* To review patient medications to enhance compliance
* Co‑ordinate the provision of travel medicine services
* Maintain accurate clinical records in conjunction with existing legislation
* Ensure read codes are used effectively by all member of the nursing team
* Ensure appropriate use of the chronic disease registers
* Prioritise health issues and intervene appropriately
* To support the team in dealing with clinical emergencies
* Recognise, assess and refer patients presenting with mental health needs
* To implement vaccination programmes for adults and children

Newent Doctors Practice is a ten general practitioner training practice with approximately 12,000 patients, based in the semi-rural town of Newent, which is just 8 miles from the city of Gloucester on the Gloucestershire/Herefordshire Border.

Experience
* Experience of working within a nursing team
* Experience of working as a practice nurse or community nurse
* Experience of chronic disease management
* Wound care/removal of sutures and staples
* ECGs
* Venepuncture
* New patient medicals
* Requesting pathology tests and processing results
* Understanding requirement of PGDs and associated policies
* Women's health (cytology, contraception, etc)
* Immunisations (routine, childhood and travel)
* Chronic disease management (Diabetes, Asthma, COPD, etc)
* Experience in infection prevention and control
* Ability to record accurate notes
* Experience of working in a GP Practice
* Knowledge of health promotion strategies
* Knowledge of public health issues in the local area.
* Mentor or teaching qualification
* Post graduate diploma or degree (Chronic Disease Management)
* Nurse Prescriber
* Family Planning qualification
