Advanced Nurse Practitioner
Working alongside the existing team you will be responsiblefor providing on the day care to a population of approximately 5,000 patientswho present with a range of undifferentiated illnesses and injuries. You will independently assess, diagnose andprescribe for these patients and undertake clinical appointments, telephonetriage and home visits with mileage reimbursed.
We are looking to recruit into a position covering Monday & Tuesday each week
Main duties of the job
The post holder will need to be an experienced Advanced Nurse Practitioner who, acting withintheir professional boundaries, will provide care for the presenting patientfrom initial history taking, clinical assessment, diagnosis, treatment andevaluation of care. They willdemonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills, for patients within the generalpractice. The post holder willdemonstrate critical thinking in the clinical decision-making process. They will work collaboratively with thegeneral practice team to meet the needs of patients, supporting the delivery ofpolicy and procedures, and providing nurse leadership as required. In order to work at this level NMC (Nursingand Midwifery Council) requirements for advanced practice must be met.

Key responsibilities
* Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with anundifferentiated diagnosis.
* Clinically examine and assess patient needs froma physiological and psychological perspective, and plan clinical careaccordingly.
* Assess, diagnose, plan, implement and evaluateinterventions/ treatments for patients with complex needs.
* Diagnose and manage both acute and chronicconditions, integrating both drug and non-drug-based treatment methods into amanagement plan.
* Prescribe and review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and within scopeof practice.
* Work with patients in order to supportcompliance with and adherence to prescribed treatments. Provide information and advice on prescribedor over-the-counter medication on medication regimens, side-effects andinteractions.
* Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care.
* Support patients to adopt health promotionstrategies that promote healthy lifestyles, and apply principles of self-care.
* Undertake vaccinations for adults, within ownscope of practice, and according to local and national policies and procedures.
* Utilise and demonstrate sensitive communicationstyles, to ensure patients are fully informed and consent to treatment.
* Communicate effectively with patients andcarers, recognising the need for alternative methods of communication toovercome different levels of understanding, cultural background and preferredways of communicating.
* Maintain effective communication within thepractice environment and with external stakeholders.
* Act as an advocate for patients and colleagues.
* Recognise and work within own competence andprofessional code of conduct as regulated by the NMC.
* Produce accurate, contemporaneous and completerecords of patient consultation, consistent with legislation, policies andprocedures.

Qualifications
* RGN
* Independent/Supplementary Nurse Prescribing
* Post Graduate Diploma: Nurse Practitioner in Primary Care or equivalent. Qualified to Degree Level
* Previous experience in Clinical Management involving first contact with patients with minor illness
Experience
* Advanced assessment skills incorporating: history taking, clinical examination, diagnosis and initiating programmes of care
* Primary Care experience
