An Excellent opportunity has arisen within Middlesbrough, Redcar & Cleveland Community Services. We are looking for skilled and outstanding senior clinicians to join our Urgent Community Response (UCR) service within the Growing the Friarage and Community Collaborative.

Non-Medical Prescribing is essential.

If you possess innovation, dynamism, drive and are passionate about patient focused care, this could be the challenge you are looking for.

You will have significant post-registration experience, be a non-medical prescriber and have advanced clinical skills to help you effectively manage a caseload of patients with highly complex needs. You will utilise your advanced clinical skills, non-medical prescribing and case management approach which will have a direct and positive impact on the persons ability to cope with an acute presentation with the aim to reduce unnecessary hospital admissions.

Experience of working in a community setting, working within an emergency assessment unit or rehabilitation would be an advantage and we welcome informal visits to the areas.

Main duties of the job

In the role of the Community Clinical Nurse Practitioner you will be a key member of the unplanned Urgent Community Response Service providing an in depth assessment of nursing care and treatment for patients in their own home to prevent unnecessary admissions and/or facilitate timely discharge from hospital.

Hours of operation is 24 hours per day over 7 days per week within the UCR service.

The successful candidate(s) will join a team of UCR Clinical Practitioners with a wealth of clinical knowledge and experience who will support and encourage you in your new post.

Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.

Qualifications & Training

Essential criteria

1. To be an independent non-medical prescriber with demonstrable experience
2. To have completed the advanced physical assessment and disease management skills course (level 7) OR have completed the physical assessment and disease management skills course (level 6) AND be willing to work towards the level 7 course within the first 2 years of appointment.
3. Teaching and mentoring qualification
4. To hold a Diploma or Degree awarded by professional body.
5. Registered as fit to practice by the Nursing and Midwifery Council (NMC) as a registered adult nurse.

Knowledge & Skills

Essential criteria

6. Participate in regular clinical supervision
7. Able to assume additional managerial responsibility and prioritise this accordingly
8. Demonstrates an ability to work on own initiative as well as part of a team.
9. Effective people management skills
10. Knowledge of clinical governance and risk assessment/management

Experience

Essential criteria

11. Demonstrates enhanced clinical knowledge and skills relevant to role.
12. To display effective communication skills, communicating complex, sensitive and contentious information to others
13. Demonstrates excellent organisational / delegation / prioritisation skills.
14. Able to evidence team leadership experience
15. Evidence of post graduate training/ experience in leadership
