We are looking for a highly motivated and enthusiastic individual to step into a Band 3 discharge coordinators post on Mulberry ward. The post holder will be responsible for ensuring safe, timely, and effective discharge of women and newborns from the postnatal ward into community care.
Main duties of the job
The role focuses on coordinating care, improving patient flow, maintaining safety standards, and supporting multidisciplinary communication to optimise the discharge process. Vital Skills to have, excellent communication skills, good organisational skills, IT skills, a good team player and the ability to problem solve.

Essential criteria
1. Foundation Degree in Health and Social Care or equivalent
2. Must provide proof of successful completion of a Foundation Degree in Health and Social Care or equivalent before commencement of the role
Experience
Essential criteria
3. Has experience working in an acute care setting, completing clinical skills to NVQ Level 3 and FdA standards. Can provide direct patient care, assist with procedures, and record patient observations accurately, with skills checked through assessments or supervision.
4. Has experience working across multiple specialties within healthcare, evidenced by examples of adapting to different clinical settings, supporting varied patient groups, and collaborating effectively with multidisciplinary teams. Competence is measured through documented work experience, feedback from supervisors, and evidence of successful completion of relevant clinical tasks across at least two specialties
Desirable criteria
5. Previous experience in a maternity unit setting as band 2 or equivalent evidenced through examples of supporting patient care, undertaking relevant administrative or clinical support duties
6. Demonstrable experience of dealing with patient and relative concerns or complaints, evidenced within the application through examples of responding professionally and sensitively, resolving issues where appropriate, and escalating matters in line with organisational policies and procedures.
